Your records are what let your provider connect your symptoms and injuries back to your military service. Below is a quick overview of the records that are useful for the process. Click any card to open a guide for obtaining those records.
Not sure where to start?
What the VA has already granted, denied, and rated.
The documents that prove what happened during your service.
Your official discharge document from the military. It shows your dates of service, branch, character of discharge, awards, and the summary of your time in uniform. This is the first document we need before we can review your case.
Your medical files from active duty: sick call notes, injury visits, physicals, profiles, immunizations, and your separation exam. STRs are some of the strongest evidence you can have for a Nexus letter.
Your treatment history from active duty, the VA, and private providers.
All the medical notes, exam results, decisions, and documents the Department of Veterans Affairs has created about you after you left the military. You can download almost everything from your VA.gov account in one session.
Everything you received outside the VA: civilian doctor visits, hospital and ER visits, chiropractic care, physical therapy, mental health notes, sleep studies, cardiology, orthopedics, imaging centers, and any other non-VA treatment.
Not required, but they help your provider give the strongest opinion.
