How to Get Your Service Treatment Records
Service Treatment Records are your medical files from active duty. These include your sick call notes, injury visits, physicals, profiles, immunizations, and your separation exam. These records show what happened to you while you were serving. They help your provider prove when your symptoms started and how your condition began. STRs are some of the strongest evidence you can have for a Nexus letter.
For help obtaining your STRs, follow the instructions below, or you may contact your Records Specialist for help walking through obtaining your records.
