Personal statement
Your personal statement is your own written account of your condition. This is your chance to explain what happened in your own words. You should describe when your symptoms started, how they began, how they affect your daily life, and what has changed over time. You can talk about pain levels, limitations, flare ups, sleep problems, mental health effects, or anything important that doctors may not have documented.
You do not need to write anything fancy. Just be honest and clear. One to two pages is perfect. You can write it on your phone or computer, save it as a PDF or Word file, and email it to us directly or upload it with your records. This statement helps your provider understand your story so they can give the strongest and most accurate Nexus opinion.
Other supporting documents
Other supporting documents help your provider understand what people around you have seen and how your condition affects your daily life and your work. These include:
- Buddy or lay statements from people who witnessed your injury, lived with you, deployed with you, or noticed your symptoms over time.
- A work impact letter from a supervisor or coworker describing any limitations you have on the job, missed days, or changes in duties.
- A symptom timeline — a simple list of key moments such as when your symptoms began, when they worsened, and any important medical events.
- Photos if you have visible conditions such as scars, swelling, rashes, or mobility equipment.
